That could either be a reaction to the injection, or some kind of indication. I work in health care too, and upon my first tb series, you receive two separate sticks. Then, each year, you get a single injection test. I do know people at work who simply can't receive this injection because they do have reactions to it. Right after the injection is given, I know it forms what is called a "wheel", which puffs up your skin, but typically that doesn't remain swollen. Perhaps call your doctor and describe your situation, and then see what he/she may want you to do further. Hope this helps to some extent.
