Annual physical?

Okay, this might sound as if i've been living on another planet but...

I'm 22 years old. I've been in a relationship with only one boy since I was 15 years old so hopefully this lessens my chances of anything icky, but...

I've never had a complete physical exam. The closest was a couple years ago when I had an anxiety attack during finals week at school and they checked my blood, heart rate, lung capacity etc. They were mainly checking to see if I was having a heart attack or had athsma or diabetes or something to cause me to feel so panicky. Anyways, I've never had a pap exam, and my roommate said you're also supposed to get an annual "physical"? What is this? I don't think I've had anything like that since primary school. I rarely go to the doctor unless I'm really ill, so I don't have a "family" doctor. The birth control center here will not renew my depo perscription without me getting an "annual". So what do I do? There is a hospital right by my house, can I just make an appointment there? Or do I need to go to a special clinic? I tried to go to a place called the Woman's health center and they were very snooty and said I needed another doctor's referral to go there. Help if you can please!!
