Labia Minora Reduction Surgery Insurance Problem

HELP!!!! I have recently undergone surgery to reduce an extremely large and deformed left labia minora. I addressed concerns about discomfort and whether or not my insurance would consider the surgery cosmetic at the time of my OBGYN visit. I was told that there was usually a case for discomfort to be covered. When her assistant called to schedule the surgery, I was informed that it would be done at the hospital and that the paperwork had been processed with my insurance company. She said that it must be covered, because there was no co-pay, etc. listed and the file had gone through. Now it is 1 month later, the hospital proved unnecessary due to a change from general to local anesthesia, and I just received an "explanation of benefits" from my insurance company. It lists an expense of $1,180 to my doctor's office. The code listed for the procedure is cosmetic, and it states that I am fully responsible for the costs. I don't even think the hospital bill or follow-up visit has been recorded yet. I was misled every time I asked about this situation prior to surgery. Now the procedure has been done, and I am suffering from the doctor's office's misunderstanding. I am not a medical professional. How was I supposed to know? It is my understanding that the doctor determines whether or not the procedure is cosmetic, so how do I convince the insurance company now that my doctor filed the wrong information? I never would have had this done immediately if I had not been told it was covered. It was more than just cosmetic. I thought I had made myself very clear. Is it my fault if the doctor didn't listen? PLEASE HELP ME!!!
