bleeding for 4 weeks with blood clots

I am 21 years old and have always had irregular periods. For the past three years I've gotten my period about two or three times per year. I hadn't gotten my period in 6 months (no chance of pregnancy) so I was about to go see my doctor. I started bleeding mildly around January 1st and I was relieved at first, but now it hasn't stopped (today is the 31st). About 5 days ago I started having these blood clots (which can probably fill a tablespoon) show up on my tampon every time I changed it. I've been having to wear a super tampon and a pad, and have had to change them both every 2 to 3 hours because they are soaked. The clots are dark red and the blood is bright red. I do not feel faint or dizzy, and everything's fine except for this inconvenience of bleeding heavily. I have never taken birth control pills, have never been pregant, and do not exercise excessively. I'm not sure what's wrong with me, but I know it can't be normal. Can anyone please help?
