17 year old with low pelvic pain

my 17 year old daughter was diagnosed with pcos a year ago with fasting glucose of 90 but has had success with a diet change and has a fasting insulin of 12.5 but had an appendectomy in january. at that time several hemorrhagic cysts were found on both ovaries and she still suffers from lower pelvic pain, nausea and now has a rectal discharge without bowel movements that appear semi-solid white and about the size of a golf ball. does anyone know what this might be. she is scheduled to see a surgeon soon but i am concerned about this discharge. i work at a hospital and have never heard this one before. thanks, martha
