What is the likelihood that i will not be able to have children? Boy, I am giving an educated guess here, but I believe that after one episode of PID, it is 15%. However, that is the background infertility rate, so I may be a bit low.
>Have i ruined my chances because this disease was in my body so long? No, you have not, again as a best guess. The easiest way to find out is to try to get pregnant :). Normally, until you are unable to get preg, testing isn't routinely done. A hysterosalpingogram, an xray dye test, would be done to check the fallopian tubes. If normal, that usually means that there isn't any damage. Also, a laparoscopy could check things out, too.
