Your doctor may look on the outside by spreading the labia and inspecting (looking at) the outside carefully. If a full exam is done, well, a device is used to looking into the vagina, and the doctor may use a finger (gloved) to feel inside the vagina while pressing on your abdomen. I bet that somewhere in obgyn.net there may be a blurb on your first exam.
