An annual pelvic exam would serve at least one purpose: early detection of ovarian cancer. Ovarian cancer is usually not caught early, hence the high mortality rate. I think the PAP smear requirement might matter patient to patient. If a virgin, your chance of cervical cancer is much lower, so PAPs might only be done every 2-3 years after a normal one. If you have more risk factors for cervical cancer, a yearly PAP is probably advisable (I'm not a doctor however).
