In an otherwise healthy 12 year old, 18 mgs is probably going to be enough, and a blood test will be able to tell you if it is not. An adolescent female is going to need about 1000-1500 mgs of calcium per day, so 400 is a part of that picture, but if there are no good dietary sources, that may not be enough.
