You need a full workup - female hormones, other hormones (such as thyroid which can cause abnormalities of the cycle and problems to be confused with PMDD). As far as the fibroid, if it is undergoing degeneration (outgrowing it's blood supply) it can be quite painful. This may stop on it's own or it may require medication or surgery to help with this last problem.
