Anything you and your doctor discuss, and any exam is strictly confidentilal. I worked family planning for 4 years, and always made it a point to let my patients know that anything that happened in the clinic was strictly confidential. The only instances where it would be reasonable for this to be broken would be something like if the patient had a really sever pap smear and did not seek treatment. So your answer is no, mom does not need to know from doctor that you are having sex, but it might not be such a bad idea to discuss it with her.
