Let's look at one thing at a time since I don't think there is necessarily just one thing going on. Blackouts or syncopal episodes have many causes - among them low blood sugar, low blood pressure and psychological/psychiatric reasons. The gingivitis is usually associated with less than perfect oral hygiene although is can be from some medications. As far as her being irritable, depressed and with insufficient weight gain, this needs to be referred to her OB. Insufficient weight gain may be due to nausea and other gastrointestinal complaints in the first trimester which can als cause irritability and depressions but may be something of greater concern for which her OB may need to be looking, as well as the possible reasons for the syncopal episodes.
