Laparoscopy is less painful and does have a faster recovery, but there is a greater chance of rupturing the dermoid. The stuff inside the dermoid is very irritating to the pelvis and abdomen, and can cause all sorts of problems. So, the real question is how good a lapraroscopic surgeon is your doctor, and can you live with the results if the cyst is opened inside of you. To put it in perspective, I am a crackerjack laparoscopic surgeon, and I am quite hesitant to try to remove dermoids that are 5cm or better throught the scope, and I am quick to abort the scope approach once we start if the conditions are not good. Still, I do usually start with the scope, just on the hope that we can take care of it that way.
