severe cramping and clots

I have always had bad cramping and heavy, long periods. I have also had painful intercourse with bleeding after, ever since becoming sexually active at 18 (also pelvic exams/paps have always been very painful). I have tried several types of bcps depo provera. None provided complete relief. My cramping has gotten unbearable and I now have intermittent sharp pain. My flow is very heavy with large clots (overflowing the pad instantly). It has taken 4 bcps a day for several days to stop the bleeding. I have had an ultrasound that showed lots of fluid, an endometrial biopsy that showed no abnormal cells and very little lining. My doctor's are not usually forthcoming with info.I want some advice on what to ask the doctor. I know that I want an answer and that I can not live this way but I am not sure what to push for. I have many symptoms of endometriosis but I know the diagnostics are invasive. I also have bowel problems (prev. diagnosed IBS-but now having more tests). What are the chances that the gi problems are linked? My husband and I want to have children but we are worried about my fertility. We'd like to wait but don't know if we can or should. Is it possible this is just after effects of being on the depo? Any advice would be greatly appreciated. Thanks.
