Re: LeepFrom: D. Ashley Hill, MD (anonymous@obgyn.net)Tue, 12 Sep 2000 06:06:59 -0500 (CDT)
Wendy- This is a tough one. Without looking at the colposcopy and knowing more about your "social" situation I can't really comment on why a doctor might do a LEEP for someone with ASCUS. For example, if a patient was going to lose her insurance, move out of the country, or would refuse to come back to the doctor, it might be valuable to perform a LEEP or cryotherapy to prevent later recurrence. Further, if someone had multiple Paps return as ASCUS or Low grade, then a LEEP or cryo might be indicated. Some doctors might feel that either of these procedures are indicated for low grade Paps on a routine basis, but I am generally not one of them. A vaginal ultrasound might indeed be useful for measuring your cervical length. However, to my knowledge, nobody knows at what length it is necessary to put a stitch in (called a cerclage) to keep a baby from falling out during pregnancy. If your cervix were, for example, half it's normal length, I would sure watch it closely during pregnancy with internal exams and ultrasounds, and may even suggest a cerclage, depending on the exact situation, but I don't think anyone really knows what length is too small. My advice is usually to either get a second opinion, or obtain a vaginal ultrasound during the first trimester of pregnancy. Best wishes,
