I can't speak for all gynecologists, but it would be very unusual for a gyn to tell a patient's mom that she was sexually active (outside of the legal requirements to do so when minor children are victims of sexual abuse by someone older). I usually meet with my first-time younger patients with their mom, then I "kick out" the mom so I can talk with the patient. I explain that even though I think most parents are the best resource for young adults to speak with, I understand that it's hard to talk to your mom or dad about sex. However, it should not be hard to talk to a doctor about this, because we deal with these issues all day long. Her doctor will likely notice the hymen is not intact, but to be honest, many things, like riding a bike, can "break" the hymen. It's entirely possible the doctor will not even notice that your girlfriend has had intercourse. There is no way to determine if someone masturbates by doing an examination.

I encourage your girlfriend to see a gynecologist fairly soon. Young women should see a gyn when they begin sexual activity. If they have never been sexually active, then age 18 is a good time to start.

You did not ask about "safe sex," but please keep in mind that there is a huge amount of misinformation going around about sex, pregnancy, sexual infections, and AIDS. The bottom line is that like lots of things in life, choosing to become sexually active brings with it a lot of responsiblity, including the potential for pregnancy and sexually transmitted diseases.
