Irregular bleeding

In February of l999 I had an ultrasound done as I was having irregular menstrual bleeding. I am 43 years old. My left ovary measured 28 x 24 x 2l mm., and the right ovary measured 41 x 28 x 27. A tiny Nabothian cyst was found and removed.

I was asked to return to the office for a pelvic as they were concerned about the right ovary being larger than the left. I canceled the appointment as the irregular bleeding stopped after the Nabothian cyst was removed. (This was all done through the ANP assisting the OB/GYN doctor).

The ANP told me to call the office should the irregular bleeding occur again. Meanwhile the ANP left to further her education. My irregular bleeding resumed again in the fall. I had bleeding after intercourse, and sometimes two periods within a seventeen to eighteen day time-frame.

I called the office and they scheduled an appointment with the OB/GYN physician. Upon examination she did a very brief pelvic, found tenderness on the right side, and told me she felt that was muscular pain, and that I was peri-menopausal (the right side was very tender upon palpation). The physician never made note of the ultrasound done back in February and suggested I go on progesterone to regulate the bleeding.

I didn't go on the hormone as I am not sure this is perimenopausal or hormone-related. My husband and I had intercourse a week ago and I had five days of light bleeding and spotting--I had just finished a period five days previously, so this was not another period. I also had discomfort on the right side during intercourse.

My questions is what should I do? Should I call the physician and explain to her I don't mean to sound like a hypochondriac, and I don't want to make more out of this than what it may be, but I want more reassurance that this is just perimenopausal. Could it be dysplasia that is causing the vaginal bleeding after intercourse? Why am I feeling discomfort on the same side as the larger ovary?

Also, I have a right congenital defect of the right kidney---a hydronephrosis---this becomes colicky whenever I have a period. What would the connection be there?

I appreciate your thoughts on this. Thanks very much.
