Bacterial vaginosis / estrogen vaginal cream

Hi I am 37 and had a tah with removal of one ovary last September. I am on premarin estrogen cream due to excessive dryness. I saw my doctor and was diagnosed with bacterial vaginosis. She said not to put anything in the vagina while on the antibiotic. The problem is that since being off teh estrogen cream I have been experiencing migraines and breast tenderness. Is this normal and can I still use the cream while on the antibiotic. Thank You Camille
