Endo in a scar is very rare, but it can happen. I just saw a patient yesterday with endo in her belly button. It can happen to anyone with a scar that is exposed to endometrium, whether from endometriosis, or from a uterine proceedure such as a C/S, where the endometrium is penetrated. The treatment is scar revision, ie reopened and the abnormal tissue removed.

You are generally wasting your time if you remove the uterus, and leave the ovaries if you are trying to treat endometriosis. The ovaries are the source of the estrogen that "feeds" the endometriosis. If you leave the ovaries, the return of endometriosis is virtually certain.
