"When you hear hoofbeats in the park, do you think of horses or zebras?" In medical school, this was a common comment. It means "do you think of common diagnoses (horses) or rare diagnoses (zebras)." This is sounding like a "zebra" and much too complex for the internet. It seems reasonable to evaluate the RF and ANA with a rheumatologist and have the rheumatologist and a gynecologic endocrinologist communicate to find the answer to this "zebra" of a problem.
