I need advice regarding a case of CAH. The individual has been brought > up as a boy. H e is completely phenotypically male with a moustache, > beard, and a penis about 3 in. long. He is 9 yrs old but looks taller. > Testosterone level are 14. An US shows the presence of a uterus and > ovaries. Karyotype 46XX. He is now complaining of lower abdominal pain ? > cause? menarche. The parents want a TAH+BSO and would like to rear him > as male. I feel that his sexual function as a male will not be normal > therefore this may lead to severe psychological problems in the future. > The family belongs to a very conservative society where reverting to a > female phenotype might be equally difficult. Please advise > Nassera
