Is continuing ed required for MD's?

It is my understanding that a doctor has to get a license for each state in which s/he will practice, and also must pass some sort of exam/internship to become board certified in their specialty.

Once a doctor is licensed and board certified, are there any other requirements to encourage a doctor to keep up with the "latest and greatest"? Do state licenses or board certifications expire?

I just wonder if a doctor who was licensed and board certified in the 60's is in any way required to keep up with the new information that continually becomes available? I assume that most do because they want to be good at their jobs and they have a genuine interest in medicine. But I would also think that anyone who has been doing a job for a long time could very well get set in their ways and not be particularly interested in the new stuff.
