After Ovulation Pain and Passing Small Clots (Second Inquiry)

-------------------------------------------------------------------------------- For the past three months on the 21-22 day of my cycle, probably after -------------------------------------------------------------------------------- ovulation my abdomen gets bloated, pressure and then it hits. For one -------------------------------------------------------------------------------- day I spot, pass clots the size of a walnut and my uterus is cramping terrible, sort of like childbirth contractions. My dr. didn't seemed concerned about this however, I am. I want to know what is causing this and if I should seek further medical attention. I am 36 years old with one child. Also, I have been diagnosed with a small 3 cm fibroid tumor and a cyst of each ovary. This was diagnosed via vaginal ultra sound. Should I seek an lap to confirm or further investigate problems. Also I noticed that my period this month acted strange. Started light on one day stopped the next day. Started light for two days and then bang heavy. Also, I noticed that I have had a major breakout of zits on my chin and neck. Is all of these problems hormonal? Could I being going through perimenopausal? I really wanted a second child but now I feel that my times up. Please someone respond I am terrified. My cousin says that I am probably dying and this doesn't help!!!
