Thank you, Dr. McIntosh. So far the answers I've gotten include two "never heard of it"s and one "not unusual." The person who answered "not unusual" (in a private email) did not identify him/herself as a medical professional (sorry if this sounds snotty -- it was a perfectly nice, knowledgeable-sounding letter). From my searches on the obgyn-l archives, training and practice with forceps varies just *hugely*, so I guess this isn't so surprising, but I did expect the technique to be mentioned and perhaps named, even if not practiced widely. I understand the reasons non-medical-professionals shouldn't post to obgyn-l, but this is one of the times I really wish I could.
