Sudden change in menstral cycle

Is this normal? Should I see a doctor?

Background: I'm 24 years old. When I was 17, I had an ovarian cyst rupture and hemorrage, requiring emergency exploratory abdominal surgery. Subsequent cysts that have come and gone over the years, have used birth control pills on and off as could afford to control.

Current Medications: 20mg of Paxil daily (since Oct 97) and Alesse (restarted Jan 99, 1st took that pill from Mar 97 - July 98, stopped due to insurance loss)

Symptoms: Beginning with my January period, I noticed a marked change menstration. 1) Increased menstral pain. I have NEVER had cramps, and now, I have painful cramps at start of period and mild cramps during. 2) Increased flow. Used to use 8-10 tampons and a half dozen panty liners. Current period, I have already used 25 tampons, and it has lasted 7 days (started before I was finished taking "active" pills) 3) Long history of diarhea and painful bowels both during period and between cycles.

Is this all normal changes due to age? Should I see a doctor? I don't currently have insurance and am afraid I can't afford expensive tests. Should I hold out until I can get insurance in next 3-6 months? Am I over-reacting? Could it just be increased stress?

I know this is all overwhelming. But I don't know where to turn.
