Chris,

You're absolutely right -- the barriers do not come with a 100% guarantee but it's at least worth a try. When I had my hysterectomy in March a saline barrier was used after the adhesions were removed (again) and so far, from the way things feel it did work for me. I think I have re-formed some adhesions on one side, if I sit too long or move too quickly in just the right (or wrong) way I feel that old familiar riptearpull sensation but for the most part I'm painfree. It's my guess that there are a whole lot of variables that determine if adhesions will occur, re-occur and the severity/density as well. Until there is a sure-fire method/solution all we can do is hope for the best.
