Re: For Dr. George - Q's about TL and informed consent.From: anonymous@obgyn.netFri, 8 Jan 1999 17:47:08 EST

Some are psychological. An in depth search for unconscious factors is best. Some are physical -- e.g. Ovarian adhesions giving pain. Some are hormonal because the blood supply to the ovaries can be altered. What type of tubal ligation was done? Post delivery? Interval simple 'cut and tie' under direct vision. (Open tubal) -- Lapscope with cautery or -- rubber band method. All of these things affect the frequency of side effects. In general side effects are not common. ================ I agree with the Progesterone injection. If no bleed occurs I would repeat it with oral estrogen a couple of weeks before the progesterone. ==================== The pictures are a little tough to read, but I would say that one ovary looks normal but I cannot see beneath it. I think that is the right ovary. The other ovary has a cyst on it. The stage 4 thing would not be an effect from the tubal. The ovarian adhesions probably are, but they could be from previous adhesions -- or -- from post op infection and adhesions. The hormone imbalance may be originating in the uterus. P4 + Estrogen to DX. It may be from the pituitary gland -- hormone levels. I may be from the hypothalmus -- stress. (P4 would probably lead to some sort of bleed or spotting.) Ask the questions. Make the doctor tell you everything. I usually get a second opinion when I am told of need for surgery or serious problem. Remember doctors are two legged animals. They have good days. They have bad days. Etc. L
