Estrogen/testosterone pellet implants

I am forty years old and had a complete hysterectomy in l995. I have been on estrogen-testosterone pellet implants since June of 96. I have felt great on these except for some weight gain. My question is, has these pellets ever been FDA approved in the U.S., and if so how do patients get their insurance to pay for the expensive procedure? Is there more risk to using this type of HRT?
