Pelvic Exam

I have been reading the forum for awhile and have a question. I am very nervous going to the gynocologist and have a hard time with any sort of exam. I have read that a patient should discuss their feelings with the doctor, but there are just some things I would prefer the doctor not to know because I do not want it written down in my chart. My question is, do doctors have to write down what you say or if I ask them not to write something down, do they have to and/or will they write it down anyway? It is not a medical concern I do not want in my chart but a social concern that just has to do with my nervousness at being at the doctor's.
