Dear Dr. David,

Well, just to respond to the so-called "sexist comments" that you and Jas (previous post) thought I made. Please also see the comments I made on the posting for JAS.

I would rate you as a GREAT doctor, and if you were in the area where I live I would visit you as a patient. As a matter of fact, I wish you could tell me what on earth is wrong with my episiotomy site 2 1/2 years after the fact!!!! It feels terrible.

Dr. David -- you are just the kind of OB/GYN that it seems so hard to find nowadays.

I would like to ask you a question regarding a comment you made: >"However....there are some instances where an episiotomy is necessary. I
>have had a number of deliveries where the baby's heart rate dropped to
>near zero, but the head was "right there." The patient may be delivering
>her first baby, or may be dead tired from 2 hours of hard pushing. In
>these cases, but only after I discuss it with the patient, an episiotomy
>may save 5-15 minutes of pushing time."

In such a situation as the one you mention above -- if the head is RIGHT there but the mom is too tired to continue pushing and the baby's heart rate is going down -- wouldn't a vacuum suction help get the head out without the episiotomy. I know that some doctors say that in case of an instrumental delivery you need to cut an episiotomy PRIOR to introducing the instrument-- but in the above case -- wouldn't a little suction help get the head out that LITTLE distance and not require an episiotomy?????

Re: your comment >"...might be shocked to learn that the worst episiotomy offender I have
>known in my career was a young female OB who not only cut an episiotomy
>for *every* delivery, but also cut a huge mediolateral episiotomy."

Well, I can tell you right now that I would never go to that doctor.

Again, let me just clarify things -- I choose my health care providers based on caring, compassion and skill. I made a very big mistake when I chose the doctor who delivered my child. I no longer plan to make the same mistake.

Also, I think it's great that your partners have the same views as you do on the episiotomy issue.

You must be a great group.

Thank you for your comments.
