Let's see what the medical professionals can say about this one.

This is where directed pushing comes into play. If the epidural is not turned down so the mom can feel, or partially feel, the urge to push, then the doctors and nurses tell her when to push based on her contractions or the situation.

If you watch the new show on TLC, "A Baby Story", you will see almost every woman getting an epidural. Most of the births are the same; mom gets labor either induced or augmented with pitocin, gets an epidural, lays in bed for most of the labor, is in the lithotomy position for pushing, is instructed to take a deep breath, push for 10 counts, quick breath, push for 10 more counts. IMHO, (no flames please) this is pretty sad. I feel bad for the moms who don't get to feel their baby's birth. There is nothing like it in the world! This show makes it appear as if the women's bodies are doing something entirely unnatural and *something* needs to be done to help her. Now I see why there is a trend towards homebirths again. Who wants to be treated like this, unnecessarily?
