Severe pelvic pain

I am 25, married, no children. I have always had bad cramps the first day of my period. However, in March, I had such severe cramps that I shook, had hot flashes and cold sweats, and even the tightest doubling-over would not relieve any of the pain. My cramps typically come and go like contractions, but these were unreal. I had acute appendicitis (and a subsequent appendectomy)10 years ago, and I can honestly say that it didn't hurt this bad. Well, at the time I had just changed jobs and had no insurance, so I screamed and cried hoping it wasn't an ectopic preg. (I have not been using any birth control for over a year.) After two hours, the pain subsided, and I slept. For the next two days, my ovaries were sore from the cramps.

The next several months were fine, with bad cramps, but nothing like that. I began charting my temps, and confirmed that I was ovulating (at least according to my temps), and was trying to get pregnant. No success.

A few days ago my period started at work, and here came the nightmare cramps. I was balled up on the floor in the private bathroom for almost an hour, and I would've went to the ER if I thought I could have walked to someone for help. It was that bad. Eventually they subsided, and again I was very sore (and still am)and went home.

I am going to see a nurse practitioner tomorrow (had an appt. for my annual anyway, but then the office called and said my doc had a surgery that day, and couldn't reschedule until after November.

Are there any specific questions I should ask? Any other symptoms that I might have but am missing? It is very hard for me to miss work for doc appt.s, and hard to schedule them, too, so I want to make the best of this one.
