Denise: It can take some time for the HCG to return to zero after an ectopic. The key however is not that the HCG is positive but rather what precise level is present,and on subsequent tests are the values falling??? If the value does not continuously drop, it means that there is still viable "pregnancy tissue" present somewhere!! Serial quantitative HCG measurements are especially important if the ectopic preganacy was treated by linear salpingostomy ( ie.was the unruptured tube opened and the ectopic tissue removed) or was the tube just removed??.In the former,if any viable ectopic tissue was left behind,it could continue to grow and cause a tubal rupture weeks later. A rising or persistent HCG level would suggest this possibility. Molar pregnancies are very different problems as some can become cancerous, and thus a negative HCG needs to be observed for a much longer time.
