At the age of 42 your natural fertility is lower than a teenager. Therefore, forms of contraception that would not be advised to a teenager due to low effectiveness can be used by you. You therefore have a huge variety of options and you are best to see a doctor about them.

Smoking and the combined pill do not go together well in a 42 year old woman. You would have to convince you doctor that you really have given up before he/she would prescribe it. You mentioned the diaphragm gave you UTIs and a cervical cap can be used when this is the case.
