Mary, (I'm not a doctor...just saying that up front) I'm wondering, after being through just such a pharmacalogical regime (I did that for over 10 years) why your dermatologist hasn't tried Accutane? I had cystic acne, not alot, but when I got them they would be the size of nickels and take 2 weeks to heal. Finally my Dr put me on Accutane. I can't remember if I did a 3 or 6 month course of them (starting at age 27) and haven't had a problem since. There are lots of side effects. SUPERSENSITIVITY to sun, very dry skin, raised cholesterol levels(each month I had to have blood drawn to check how my liver was functioning and to verify that I wasn't pregnant...ha! not unless a star was present in the eastern sky!), you MUST be on BC Pills(I used Lo-Ovral 21) and not even THINKING of conceiving...but I can't tell you the difference it has made in my skin. I have been pretty much blemish free, with the exception of a pimple maybe twice/year and NEVER anything that remotely resembled the cysts I had. Another factor, and it may be different for you but I was put on probably 4-7 different oral antibiotics...which all worked to a degree but when I stopped them, it was back in full force. Also, by being on so many antibiotics, I have built up an resistance to them so they are no longer effective for me at treating simple bacterial infections.
