Incompetent Cervix?

I recently had to terminate a 21 week pregnancy due to pre term labor. I had some early brownish spotting at 12weeks and after a full sonogram, was deemed to be fine. Upon entering my mid term, my doctor order another full sonogram, of which the results were the same as above. 2 weeks later, I was fully dialated, too far along to be able to do anything without risking infection and possible infertility. If I have an incompetent cervix, wouldn't it have been apparent at my sonogram 2 weeks prior? What would be the other cause of pre term labor and should there be a diagnosis rather than just a "fluke?" I should note that prior to this, my previous pregnancy was carried 2 weeks overdue, leading to a scheduled induction. (Prior, I had a miscarriage at 7 weeks)
