Weak tissue, continued heavy exercise or excessive weight, or incomplete tissue repair at the time of the initial surgery. Prolapse is similar to a hernia, and for a variety of reasons as just alluded to, it can recur. There have been about 200 operations described for urinary incontinence, so that tells you that the problem tends to be recurrent.
