Endometrioma is a pathological ovarian mass--will not resolve spontaneously.Ultrasound is not 100% accurate in this diagnosis,but helpful.If surgery must be delayed--would begin Lupron now;this will not cause resolution of this large mass,but should delay progression of disease and help with symptoms.
