A bladder or vesico-vaginal fistula is where a hole develops in the bladder and the urine leaks through to the vagina resulting in continuous uncontrollable incontinence. Most commonly it follows pelvic surgery. It is rare following colposuspension and can usually be completely excluded by simple tests. Treatment may be by catheterising the bladder for several weeks to see if it heals spontaneously or more likely surgery to close/repair the defect.

An unstable bladder or detrusor instablity is where the bladder muscle contracts inappropriatly during fillling of the bladder when it is meant to be relaxed (or stable.) It is a common cause of urinary frequency, urgency and urge incontinence and may sometimes occur for the first time follwing otherwise successful continence surgery. The best treatments are bladder retraining ( there are many other names for this) where you teach your bladder to obey your wishes rather than the reverse! or anticholinergc drugs such as propantheilne bromide or oxybutynin. Sorry I don't know the US names.
