Without the benefit of an exam, it sounds like hidradenitis supperativa. Mainly found in the genital and axillary (armpit) area, they have a tendency to recur and antibioticx therapy is the best available. As you indicated, surgery is relatively ineffective.

Unfortunately, no. Aggressive antibiotic therapy at the beginning of an outbreak is the best therapy today.

This may be from scarring from old lesions.
