A part of the story is missing and I know you are not in possession of this part. If it were Mittelschmerz, the pill would have cleared it. 4cm cysts don't usually cause pain because pain is usually caused by torsion (twisting) and the usually don't twist til 5-6 cm or larger.

Most gyns feel that if the cysts don't shrink by three months, it's unlikely that they will. Remember that there are exceptions to every rule, however.

The diagnostic test would be a diagnostic laparoscopy and then resection of ovarian cyst if necessary.

As far as which test is better, the sonogram is usually more revealing than the CT scan. There are, unfortunately, exceptions here too, but as a rule, the sono is better.
