A normal CA125 is a good sign but unfortunately not a guarantee. as I wrote in the last post, the CA125 is nonspecific and has its best use as an indicator of response to therapy in patients who already have ovarian cancer.50% of Stage 1 cancer of the ovary has a normal CA125. BTW, there are different cell types causing cancer of the ovary that do not elevate CA125 at all. In addition, fibroids and endometriosis can raise the CA125. The cyst is not large enough to mandate surgery based solely upon size but the concept of "complex mass" is of potential concern. Most often, the radiologist can give an educated opinion as to the possible pathology involved. This is not a guarantee but rather an impression that helps ob-gyns manage these problems. If the radiologist thinks it's benign, I'd have a tendency to wait. If he/she's concerned, so am I and I'd schedule surgery sooner rather than later.
