Breast tenderness/pain -- PMS or something more?

I've always had very painful periods due to cramps mostly, partially because of the irregularity of my cycle. For three days now, however, I've been experiencing not only a swelling in my breasts, but almost constant discomfort bordering on pain. I can't predict whether my period will be coming soon, but I'm getting the other PMS symptoms I experience -- stomach cramps, irregularity, etc. However, these symptoms have never lasted more than a day before starting my period.

I'm 20 years old, and because of lack of health insurance I have never had an examination by a GYN. (I'll begin my coverage through work next week and I plan to make an appointment, but the female doctor I desire an appointment with is backed up for two to three months) It's been two months since my last period, and I haven't had intercourse in nearly two years. I'm not on any prescription drugs or birth control.

The pain in my breasts is getting unbearable to the point of interrupting sleep. I need to know if this is a serious problem, or just my PMS getting the best of me. Any response appreciated. Thank you.
