It can be anxiety provoking to go to the doctor. Let me tell you what the first visit usually involves. It may be different for your provider, but I like to get all of the history, physical exam, and lab work done at the first visit. The first thing I do is talk with the patient. We discuss prior history of medical or surgical problems, prior pregnancies, medications, allergies, family history, and social history (ie, job, marital status, smoking, ethnic background, etc). The patient usually changes into a gown and an examination is done. I examine from head to toe. The last part of the exam is a pelvic. If you have not had a pelvic before, I can explain how it is done. The first part is a visual inspection of the external genitalia. The second part involves a speculum examination of the vagina and cervix, cultures and a pap smear are usually performed. The third part of the pelvic exam involves placing one or two fingers into the vagina and a second hand over the lower portion of the abdominal wall to fell the size and shape of the uterus , tubes, and ovaries. Some do a rectal exam at the same time. The provider should explain each step of the exam. Some docs like to do an ultrasound on the first visit. After the exam, I like to have the patient dress. Then we can discuss any important issues that I or she need to discuss. Specimens for routine labs are usually drawn and a follow-up visit is scheduled...
