I'm wondering about the use of perineal support, perineal massage and manual stretching of the perineum and labia just prior to delivery of the fetal head. If the tissues of that area are physiologically designed to stretch and are doing so, shouldn't they be left to do their work? Does the insertion of fingers compromise blood supply to those thinning tissues? Every time I've seen sretching and massage being done, the tissues have become swollen and invariably tear. I'm not suggesting that they might not have torn otherwise, but I wonder about the rationale for massage and stretching.
