Dr. Jick gave a superb overview of shoulder dystocia and Erb's palsy.

One additional note. Erb's palsy has been noted in babies born by Cesarian section and in many instances can be an antepartum (prior to labor) event. This was written about in a recent review article in Contemporary Ob-Gyn.
