Failure to bleed after a progesterone withdrawal test indicates a relative lack of estrogen or a problem in the uterus. The next step in your evaluation is to try and differentiate between the two. The test involves providing the uterine lining with estrogen to simulate the follicular phase of your cycle and then add the progesterone to simulate the luteal phase of your cycle. If there is a withdrawal bleed it suggests that estrogen deficiency is the cause of your amenorrhea. If there is no bleed it suggests an anatomical abnormality that is is blocking the flow of blood.
