UNSURE: concerned and in pain

Hello I have questions and concerns. I am 30 years old. I have two children and my tubes are tied. Three years ago I was having right side pain and I went to ER. They discovered that I had a cist on my right overie. I did not have insurance at the time so I could not do follow up care. I have contiuned to to have pain in my right side along with my left side now too. It also hurts to push on my lower stomach (where the uterus lays). I also do not have normal cycles. Sometimes I only have two a year. Here lately though I had a cycle that lasted five and a half weeks. I was given provera to stop it. It did finally stop the cycle, but I started spotting a few days after that all over again. That was a week and a half ago now and I am still spotting. I had a Pap test done and it came back in normal range. I did have a pap come back a few years ago that was not in normal range. I also just had a ultra sound and it did not show any overie cysts, but my uterus was inlarged. The doctor did a uterian biospy and she said it came back fine. Something has to be wrong some where becasue I hurt all the time it is worse when I am having a cycle. When I am having a cycle I also get shooting pains in to my privates. I also don't have normal cycles. Over the last two months my bowel movements have changed too. I use to go once a day, and now it hits me like direaha, but it is not it just is not a formed movement. I am worried, the pain effects me working and taking care of my kids. I have to lay on heating pads all the time just too be able to sleep. Does anyone know what this might be or what to do next. Please help.
