Message posted on Neena Agarwala, M.D. behalf by: Hope Waltman, Fibroid and Women’s Health Forum Moderator Dear Heba, Chocolate cysts are endometriosis on the ovary and you will have to use some form of suppression of menses and therefore endometriosis to keep it in check. Unfortunately, it has come back for you - either because it was not removed completely or a new one has formed. If it is very painful, you will have to have surgery to remove the cyst or even ovary (since this may have totally ruined your ovary now - if a second large endometrioma)and then be on continuous low dose OCPs such that you have periods may be every 6 months.
