UROGYNECOLOGY: Post-op and urinary incontinence

Hi, to make a long story short, I had a Tot sling cystocele repair in May 07, developed rectocele and enterocele, surgery number 2 September 07 rectocle w/graft and enterocele repair. Had permanent left sided groin pain since surgery number 1, it got worse. I went through nerve blocks, interstim sacral nerve stimulation (in an efford to control pain/not for urge incontinence) and finally partial sling removal in March 08. I had sling erosion. Ever since the sling removal I have stress incontinence again, but also developed at first bladder spasm and pain, but now really bad urge incontinence with complete loss of bladder control. I have tried Vesicare, Enablex and Sanctura XR to control urge incontinence, none of them with success...I am still taking Sanctura right now, but when I take a look at my voiding diary, I have on average 12 trips to the bathroom during the day, plus on average 14 leaking episodes, 2/3 urge incontinence and 1/3 stress incontinence..... I have no problems at night, no urgency or frequency. Its seems as all my problems are related to me being upright and active. I have no control over my bladder at all, when going out in public I am forced to wear diapers. I am only 36 with two young kids at home. I have no other health problems. I live in a very small town and my last two surgeries were at a bigger hosptial in a teaching setting. My current doctor is 3 hours away and we communicate via email/phone/txt message. I have not seen him in three months. My question, is it possible that some sort of new prolapse (I still have my uterus) could cause the new onset of urge incontinence? I had urodynamics testing done in his office three months ago. I was told I had enough bladder decent to cause stress incontinence and urethral hypermobility and sphincter deficiency. During urodynamics it was also discovered that I had urine retention. Upwards of 200 ml PVR. I had suggested to try self catherization, but my doctor does not want me to do it, as he thinks once started it would be very hard to retrain my bladder. Back then I was told it was very difficult to manage all of these bladder problems. Since then I have even more symptoms and I get the feeling that he has not much to offer anymore.... Is there really nothing other than the drugs out there to fix urge incontinence? Aren't there other people out there that have similar problems like me? Is there a possible link between POP and urge incontinence or PVR and urge incontinence? Any input, suggestions would be greatly appreciated... Thanks caitcon
